How it Works
Have you paid cash for medical services? This usually happens when you visit a service provider not on the Maisha Health Fund network who asks you to pay cash for the cost of medical services.
Submit the following documents:
- Service Provider Claim form, signed and stamped by service provider
- Attach receipt of the cash payment made
- Fill in Maisha Health Fund Claim form (here) or (download form)
- Attach prescription where applicable
Form should have principal member’s bank account details.
Maisha Health Fund will process the refund within and up to 14 working days.
Maisha Health Fund will process refund up to maximum payable for that treatment up to the member’s categorical benefit limit.
All Refund will be made into the principal member’s bank account. No cash payments will be made
